Enjoy flexible, ongoing care with the Downscale 365 Plan. This monthly subscription includes unlimited telehealth appointments (phone or video) with our Nurse Practitioner, covering weight loss, prescriptions, pathology requests, reviews, and general health concerns. All for $65 per month and we are happy to treat all issues you may be faced with.

Why a subscription model?

From November 2025, the Australian Federal Government will introduce new legislation that restricts Medicare billing for telehealth services. Under this change, telehealth clinics will no longer be permitted to bill Medicare unless the patient has been physically seen in a clinic within the previous 12 months. This means that even if a patient has received consistent, safe, and clinically appropriate care via telehealth, their clinician will be unable to continue billing Medicare unless an in-person appointment has occurred.

We believe this policy is short-sighted, inequitable, and detrimental to the thousands of Australians who rely on high-quality virtual care. For many patients — including those who live regionally, parents juggling family responsibilities, individuals with disability or limited transport, or those simply managing full-time work — this restriction places unnecessary barriers between them and the healthcare they trust.

At Downscale, we’ve created a private, transparent model that places the needs of patients above bureaucracy. It allows us to continue delivering consistent, personalised care without being bound to outdated funding models that don’t reflect modern healthcare delivery. It also enables us to remain viable as a small, patient-first business at a time when independent telehealth providers are under threat.

This model allows us to continue offering care that is flexible, responsive, and clinically sound. You’ll still be seen by the same clinician, have access to real follow-up, and receive the level of support that Medicare-funded telehealth models have increasingly failed to provide. We’re not a call centre, and we’re not here for shortcuts — we’re here for continuity, professionalism, and human connection.
